Transport for over 65’s

Eligibility

  • Residents of the Gold Coast over 65 years
  • Aboriginal/Torres Strait Islanders over 55 years
  • Approved by My Aged Care

Assessment

To receive government subsidised transport, you must be assessed by
My Aged Care.

The My Aged Care is part of the Australian Government’s changes to the age care system which was created to improve people’s choices, ensuring that they are accessing the necessary age care services with more control.

If My Aged Care consider you eligible for subsidised transport, they will do an over the phone interview followed up in many cases with a Home Support Assessment. If you are approved for transport assistance it is important that you request Transcord as transport service provider. My Aged Care will then send Transcord your transport referral and then Transcord’s client intake staff will contact you to register you.

Once your eligibility for subsidised transport has been approved by My Aged Care, Transcord’s client intake staff will contact you to discuss pricing.

If you are not assessed as eligible for funded transport, we would be happy to supply non funded transport for you.

Transport for under 65’s

Eligibility

  • Residents of the Gold Coast over 18 and under 65 – not receiving an NDIS Plan

Service Users

Whether you need ongoing assistance or just need temporary assistance while you transition back to driving our service is available to you.

Community transport service users must be:

  • Residents of Gold Coast over the age of 18 and under 65
  • Transport disadvantaged clients not receiving NDIS Services
  • Unable to access appropriate transport through other funded / subsidised schemes
  • A person with a disability
  • A person with a mental health condition
  • A person with a chronic health condition
  • An unpaid carer travelling in their role as a carer. An unpaid carer is not paid a wage or salary for care provided but may receive a carer payment or allowance

Service Type

Community transport program service type description.

Community transport provides transport services that if required, incorporate appropriate assistance based on individual service user needs to get in and out of their home and the venue they are accessing. The service focus is to enable service users to:

  • Access their community and essential services
  • Participate socially and economically to maintain their independence
  • NDIS

    Assessment

    To receive a transport allocation in your NDIS package, you must be assessed by NDIS.

    The National Disability Insurance Agency (NDIA) is an independent statutory agency.
    Their role is to implement the National Disability Insurance Scheme (NDIS),
    which will support a better life for hundreds of thousands of Australians with a significant and permanent disability and their families and carers. The NDIS will mean peace of mind for every Australian – for anyone who has, or might acquire, a disability.

    Before you attend the interview to be assessed by NDIS please ensure that you contact Transcord Intake staff to obtain transport quotations as evidence of your requirements. This should assist you to obtain the best possible outcome in your package.

    On receiving your NDIS package it is imperative that you contact Transcord to put a service agreement for your transport needs in place.
